CONSENT FORM FOR ALL THERAPIES

(Not including spa bath or wellness checks)

A consent form filled out by your vet is required before we are able to treat any animal. This ensures that we are informed of any health conditions and medication that treatment may impact upon and therefore needs to be taken into account.

It is also a requirement that a copy of the consent form is sent along with any insurance claims. Insurance companies may not cover claims without it.
